We like to think of BMI as divided into 4 groups, below 20 underweight, from 20 to 25 normal, from 25 to 30 overweight, and from 30 on obese.  Patients with BMI’s below 23 tend not heal as well.  More wound separation.  Same for patients over 30, as fat against fat does not heal as easily as well vascularized tissue.  Add to that your torso weight which could be over 100 pounds.  One hundred pounds of pressure when sitting directly on your perineum can definitely open incision lines.  The recommend ring pillows be worn at all times for 6 weeks after surgery unless sitting on the commode which works like a ruing pillow in that the support is on your lateral buttocks.  This is emphasized again and again, and some many post op patients are seen at the office and we ask “where is your ring pillow” and they reply “it’s in the car.”
