Orchiectomy with scrotal removal

is surprisingly a frequently requested procedure.  For men who are not clearly transsexual, a letter of psychiatric clearance is required and if married a notarized letter from their wife granting approval  is posted in their records.    All are informed that there are many anti testosterone (blockers)  available which pharmacologically may have the same effect, but whereas for some transgendered the removal of all or a part of external genitalia satisfies a psychological need, regardless of whether or not the patient is asexual, the same can be said for this category of men.  My personal opinion is that deep down there are common underlying psychodynamic streams which nurture both psyches.

Today we did want amounts to the first part of a scrotal vaginoplasty.  Removal of scrotal skin  and subsequent orchiectomy, bringing the margins together in the midline.   The patient was discharged quite comfortably with his wife in attendance and I’ll be seeing him at the Whitehouse (Inn on the Bay) in the morning.
