<p>While I can understand perhaps why you not be able to dress 100% en femme now, will you be doing this after surgery, and if so why would it be possible then?</p>

<p>Yes, Angie, smoking is a big enemy of a good cosmetic<br />
result, but also of your entire cardio-vascular system<br />
and lungs. Ocks.</p>
<p>We do hormones.  A letter of therapy clearance is required by a licensed<br />
therapist with a doctoral degree.  Our fee is 250 for consultation, and 600<br />
per year to follow you with needed lab tests, office visits, prescriptions,<br />
counselling, numerous telephone calls, and medical records documentation. </p>
<p>Lab tests are an out of pocket expense as is the medication.  We may<br />
recommend a laboratory, but you can have them done at a nationally recognized<br />
laboratory of your choice.</p>

A? and would like to go to a large ?B? or small ?C?  ?. guess I really want the ?C?.  It?s hard too picture.  I would prefer under the muscle because I have read there is less chance of loss of sensation which I would HATE to lose and you have stated that the feel is close?  Is this all true?  I?m 5 6 1/2 tall and 128 lbs so I am kind of small framed. I have been thinking about it a lot and I guess I have to consider my age in the equation. How much money do you stick into an older car?  LOL  The cost of the MRI?s is a major consideration.<br />

My left breast is slightly larger than my right so I guess Saline would be an advantage there. I have read that they can try to match silicone by using different pre-made sizes but that seems like it could be a bit hit or miss to me.<br />

Question ? let us say that after three years one breast deflated. Would you be able to just put one replacement in?  Failure rates are not super high to begin with but I am curious what happens should one rupture.  Brenda</p>

<p>For a lady of your height, I would recommend you gain a good 10 pounds before surgery, in that with more protein on board you will heal better and be less likely to have wound healing complications.</p>
<p>Yes, of course, if one side deflates it can easily be replaced (unilaterally).</p>
<p>For sure, you should have a subpectoral implant and I can assure you thus far we have not seen any loss of sensation, but if you are a very athletic person, you need to be pre cautioned your arm strength may be slightly affected.  Practically speaking this has not been an issue in any patients we have done thus far.</p>

<p>Our fee for silicone breast implants is 6000 which includes surgery, anesthesia and use of the facility.</p>
<p>The implants alone cost about 1000 each.</p>
<p>We can do saline for 5000.</p>
<p>There are many caveats about silicone, and I wish I could advise in detail, but larger incisions lest you break the silicone (silent rupture which manifests itself later on), not all that much different in feeling if you go subpectorally, need for MRI 2 years later (FDA and manufacturer&#8217;s) recommendations and one every 3 years, soon will outpace cost of the implants themselves, inability to make fine gradations between each breast as one could do with saline, etc, etc.</p>

<p>Silicones are proving more popular than saline. Rippling is more common<br />
in small breasts B&#8217;s and A&#8217;s with saline.</p>
<p>Detection of rupture with saline of course is easier.</p>
<p>We have seen only minimal rippling in one saline patient in one breast only<br />
and not visually noticeable.</p>

<p>Washington ­ A Food and Drug Administration review has uncovered a possible link between breast implants and a rare form of non-Hodgkin’s lymphoma, MedPage Today reports.</p>
<p>The review identified 34 cases of anaplastic large-cell lymphoma (ALCL) in women with breast implants. Twenty-seven cases involved silicone-filled devices.</p>
<p>A statement posted on the FDA website notes that agency officials are aware of another two dozen or so cases of ALCL among implant users worldwide.</p>
<p>It says that although the possible link requires confirmation, “The FDA believes that women with breast implants may have a very small but increased risk of ALCL. Because the risk of ALCL appears very small, FDA believes that the totality of evidence continues to support a reasonable assurance that FDA-approved breast implants are safe and effective when used as labeled.”</p>
<p>MedPage Today reports that in a press briefing, William Maisel, M.D., of the FDA’s Center for Devices and Radiological Health, said, “We need more data and are asking that healthcare professionals tell us about any confirmed cases they identify. We are working with the American Society of Plastic Surgeons and other experts in the field to establish a breast-implant patient registry, which should help us better understand the development of ALCL in women with breast implants.”</p>
<p>The agency also will work with implant manufacturers to update product labeling information.</p>
<p>Other review results discussed during the press briefing:</p>
<p>• Most of the ALCL cases were diagnosed after silicone implants returned to the market in 2006.<br />
• The diagnoses tended to occur a median of eight years after implantation<br />
• Most lesions arose in the fibrous capsule that formed around the implant.<br />
• A majority of cases involved implants for breast augmentation as opposed to reconstruction following breast cancer surgery.</p>

<p>Good morning Kathy,</p>
<p>Happy Thanksgiving to you and your loved ones (hope that includes me),</p>
<p>Silicones when done retropectorally do not feel that dramatically different from saline.  The incision has to be bigger, because silicones are inserted &#8220;pinch by pinch&#8221; into the incision until; it&#8217;s more than 50% in and then the rest follows somewhat more easily.  Salines on the other hand are completely collapsed, rolled like a cigarette, can be inserted into a 1 1/2&#8243; incision fairly easily which usually heals inconspicuously if the surgeon realizes that the incision line is not where you would think it should be but lower in that any breast implant lifts the incision line if done in the infra mammary fold.  Please see photographic examples&#8230;</p>

<p>Salines are reputed to fail every so often, but in our experience we have not seen one fail.  Silicone may leak, even cohesive gels can rupture, and for that reason the FDA advises MRI&#8217;s every 3 years.  Very costly.</p>
<p>To help you out Kathy, we can deduct 500 from each price (5000 and 6000), good for 1 month and to be to be secured with a 50% deposit, and to be scheduled anytime within one year from now.   Silicones cost us almost twice what salines do.</p>

Why are saline implants your first choice?  I watched this video on you tube that showed all the different implants and from that I was sure silicone was the way to go.  Is there some information that I am not receiving? I was certainly under the impression that silicone was superior.   I guess leakage or puncture is my greatest concern other than the loss of sensitivity.  Is it just the price that makes people choose the saline instead of silicone?  I am hoping to have my surgery done before the end of this year.   I certainly don&#8217;t want to make any bad decisions so if you could enlighten me a bit it would be very helpful.  How many days would I have to remain in Florida?  I would love to stay all winter but I don&#8217;t have quite that much money. XD<br />

<p>I do not know of which you speak and if I ever learned it, I am glad I have since forgot it.</p>
<p>4,800 as an all inclusive price is a good deal.  Our fee is 6000 and we are busy.  Next silicone breast implant scheduled for this Tuesday.  However, we do 3 times as many salines as silicones for many reasons, also 5000 vs. 6000.  I challenge the average lay person on palpation with a retropectoral approach to correctly identity which is which.</p>
<p>We recently redid a silicone implant from another doctor that wound up too laterally, almost 50%  under the patient&#8217;s armpit.</p>
<p>We also seen others with not the happiest of incisions, but I usually refrain from commenting for reasons you understand.</p>

<p>Please regard this as a courtesy letter to answer your concerns.  I am not trying to solicit your business.</p>
<p>Advantages of saline include, smaller incision because we put it in folded like a cigarette and expand it once in side.  There is variation in the amount of volume you can fill, for example I can &#8220;legally&#8221; put in 30 more cc or any amount in between to perfectly balance out both breasts, which we do under anesthesia when you are seated on the operating table.  Everyone awake (that&#8217;s not you) comes to the foot of the table and gives their critique of the balance and position.</p>
<p>According to FDA recommendations with silicone you should have an MRI done every 3 years as the capsules are believed, prone to rupture.  This does not mean the cohesive gel will necessarily leak out, but not a desirable situation none-the-less.</p>
<p>For the likelihood of salines rupture, please read this excerpt:</p>
<p>&#8220;Breast implants can potentially remain intact for decades in the body, but all such devices will fail at some point.  When saline breast implants break, they often deflate quickly and can be easily removed. Prospective studies of saline-filled breast implants showed rupture/deflation rates of 3?5% at three years and 7?10% at 10 years for augmentation patients.<a href="http://srsmiami.com/blog/wp-admin/post-new.php#cite_note-Walker2009-62"><sup>[63]</sup> Among the suspected mechanisms for rupture are damage during implantation or other procedures, degradation of the implant shell, blunt or penetrating chest trauma, and in rare instances from the pressure of traditional mammograms.</a><a href="http://srsmiami.com/blog/wp-admin/post-new.php#cite_note-FDA_2004b-63"><sup>[64]</sup>The age and design of the implant are the most important factors in rupture, but estimating ruptures rates of more contemporary devices has been difficult, as most previous reports</a><a href="http://srsmiami.com/blog/wp-admin/post-new.php#cite_note-Brown2000-64"><sup>[65]</sup> mixed heterogeneous groups of devices in non-randomized populations.
