<p>Yes, before proceeding to either phalloplasty or metoidioplasty you should have a laparoscopic assisted vaginal hysterectomy, bilateral oophorectomy, and colpocleisis (closure of the vagina by fulguration and purse string sutures).</p>

<p>Yes, you can do this.  However, stand to void results are more fraught with incomplete closure and may need revision surgery.   Also if you are looking for a stand to void result, the entrance way may be somewhat smaller.   So an option to consider is just metoidioplasty and void sitting as you do now.</p>

A few questions I have are:<br />
1. is it possible to do the meta and the lift together w/o the lipo?  I will just have to redouble my efforts to decrease my weight on my own.  In re-reading your email, you stated that inflammation from the lipo procedure will complicate healing in the lower area of the meta procedure.  Additionally, I have been informed by the surgeon who performed my top surgery that lipo is only as successful as the patient&#8217;s ability to maintain appropriate habits/exercise.  I believe I need to work on that first, before accessing a lipo procedure.<br />
        I can definitely see the benefit of the lift part of the procedure and would really like to have that done.  Time constraints are my biggest impediment.  As I let you know, I have a wife and a son who have to be factored into the equation with both $ and time.<br />

2. What is the length of recovery/return to normal activities associated with the lipo and how far post could you safely perform the meta, if I did choose to do the lipo before?<br />
 <br />
    I would prefer to do the lift and meta together and then at a later date perform the lipo.  Of course ultimately, I would like the scrotoplasty done as well.<br />
 <br />
Again, I&#8217;m sorry for troubling you with all of this&#8230;.as it is a major undertaking. I look forward to hearing from you.<br />

<p>Good morning Milton,</p>
<p>The lipo and the lift should be done together as this is surgery in the same general area.  Fat in the labia majora also will be removed as the fat distribution tends to resemble a horseshoe in configuration. Then you need about 3 months of rest for all the swelling to subside.</p>
<p>Your plastic surgery friend is not entirely wrong about changing your lifestyle but also once fat cells are removed substantially from an area with aggressive liposuction, that area will not become significantly plump again, as there will be very few fat cells left to store fat.   If you continue to overeat, yes other areas will become &#8220;plumper&#8221;.  Despite aggressive dieting pubic fat has been called privileged fat and is usually the last fat to disappear. </p>
<p>Most likely blousing of your skin devoid of fat will remain and of course this is why we do the &#8220;tuck&#8221; or lift.</p>

<p>My dear friend, for whom I have great respect and deep concern,</p>
<p>Yes, I perform this procedure and your fee to include anesthesia, use of the facility and any follow up care I perform is 4850.</p>
<p>Please understand the female genitalia are 30 degrees lower than its male counterpart and we are seeking to elevate the clitoris to adjust for that.  You can test this by standing in front of a mirror<br />
and lifting your abdominal panus and mons pubis and see if you like that result, aside from providing a more youthful appearance.</p>
<p>This procedure and a metoidioplasty should certainly not be performed at the same time as the pubis is the delta that lymphatically drains the clitoris and labia and temporary injury from liposuction to the lymphatics will result in more swelling and compromise the result further down by not providing an optimal environment for wound healing.</p>

Hormones/Conversion: Yes, I have been on Testosterone since Dec  2, 2002. I have had a full lap hysto and chest reconstruction.<br />
Specific_Questions: Is the vaginectomy included in the 12,500  meta?<br />
If it is then everything for bottom surgery is 12,500+5000 = 17,500?? How much extra is the mons reduction so the penis sticks out farther?</p>
<p>Can you at least tell me the length of stay in Miami for each surgery:</p>

<p>Great. Please let me know what you have available in July, the 5th Monday would be good. I wll be in Clearwater, Florida and I could drive down for the appointment.</p>
<p>Do you recommend doing the mons/publis reduction first? If so we could schedule that for late may 2011.   If you only require me to be in Miami one or two weeks for the metoidioplasty surgery I could maybe do it earlier than may of 2013.    Please let me know.</p>
<p>I can schedule the 3 sugeries separately correct?  Mons reduction first, then metoidioplasty/vaginectomy/ureathral hookup and then testicular implants last. How far in advance can I schedule surgery?</p>

<p>Sounds like you are well on your way.  Yes, we&#8217;ll be happy to include vaginectomy (colpocleisis) in your fee for 12,500.  Testicular implants are 5000 extra and I would propose that this be done as a secondary stage.  We can do pubic reduction gratis as an additional procedure, but remember the pubis is the delta which drains the lymphatics from the perineum and labia, so too much surgery there causes a swelling and sub-optimal wound healing down below.  Best to have that done as a procedure unto itself.</p>

<p>Read the letter and this is the classic example of how a letter of endorsement should not be written. This type of letter is in fact seen when a therapist feels a certain obligation to say something but in fact has deep seated reservations about making any written commitment. This is akin to a &#8220;letter of recommendation&#8221;  for an employee.   Miss so and so worked in my office from June 1st 1999 to September 5, 2001. (Start, period, end of letter).</p>
<p>This is a more a letter of detachment.</p>
<p>The standards for how a letter should be written if the Dr. is interested are posted on WPATH.   To wit:</p>
<p> </p>
<p><strong>The Mental Health Professional’s Documentation Letter for Hormone Therapy or Surgery</strong></p>
<p><strong>Should Succinctly Specify:</strong></p>
<p>1. The patient&#8217;s general identifying characteristics;</p>
<p>2. The initial and evolving gender, sexual, and other psychiatric diagnoses;</p>
<p>3. The duration of their professional relationship including the type of psychotherapy or</p>
<p>evaluation that the patient underwent;</p>
<p>4. The eligibility criteria that have been met and the mental health professional’s rationale</p>
<p>for hormone therapy or surgery;</p>
<p>5. The degree to which the patient has followed the Standards of Care to date and the</p>
<p>likelihood of future compliance;</p>
<p>6. Whether the author of the report is part of a gender team;</p>
<p>7. That the sender welcomes a phone call to verify the fact that the mental health professional actually wrote the letter as described in this document.  </p>
<p>The organization and completeness of these letters provide the hormone-prescribing physician and the surgeon an important degree of assurance that mental health professional is knowledgeable and competent concerning gender identity disorders.</p>
<p>More or less in the first paragraph the therapist should introduce him/herself by virtue of their background and training and societal memberships. Next the therapist should endorse the client for transgender surgery (&#8220;I recommend, I endorse, or Miss so and so would be a good candidate for transgender surgery, etc&#8221;).</p>
<p>Next suicidal ideation or attempts (yes or no) should be mentioned, as well as any drug abuse history or ethanol dependency (yes or no).</p>
<p>Lastly the therapist should invite the recipient to call their office for further discussion should questions arise.</p>
<p>Look at Dr. Mora&#8217;s letter hopefully and you will see the difference.</p>
