Hi  Dr. Reed,

Basically, I’m a female and my partner is a female…I don’t want to be transgendered in the speak of ‘turning to a guy’ but she does as do I, want me to have a penis so we can share that actual sexual sensation with each other. I know this email is almost a year after your response. I was able to contact Dr. M. but unfortunately he was unwilling to do a phalloplasty on me. I have been in continued therapy for this and I’ve pretty much come to the conclusion that an ordinary phalloplasty is not for me until I feel like I’ve exhausted all other options.

Was a genetic male, had a vaginoplasty and would like a penis again. 

So I have a new question for you: I know that at least theoretically, given the various organ transplants that have been in the news lately (hand, face, etc) a penis transplant is possible. In fact, they even tried it in China about 5 years ago but had to remove it because of a psychological reaction between the patient and his wife. And I know there are downsides to organ transplants such as anti immune system drugs. However, I feel the trade off – FOR ME – is worth it. My quality of life regarding what I have between my legs is awful (whereas the rest of my life is fine) so I want to try to find a doctor and a medical board willing to let me try a transplant. I know this is a truly ONE IN A KIND thing. I would be maybe the 2nd person in the entire world to have this done. It’s not something that most doctors would be able to even begin to point me in the right direction – however, I’m wondering IF you can help me in my endeavor. Can you give me any idea where to start on finding such a doctor who would be willing to do this?

Problem # 1 is to find a therapist who will say this is right for you.  Given the unusual nature of your request this could be a stumbling block.

Then look at the attached letter and choose another surgeon.  Consider Dr. Salgado, Dr. Djordjevic, Dr. Djinovic.

I’ve been on hormones for 6 years. I’ve also had top surgery about 5 years ago.
Specific_Questions: I’d really like to stand to void. I was wondering if it is better to have a hysterectomy and vaginal closure before or after my phaloplastmy especially in regards to the lengthening of the urethra.

Yes, before proceeding to either phalloplasty or metoidioplasty you should have a laparoscopic assisted vaginal hysterectomy, bilateral oophorectomy, and colpocleisis (closure of the vagina by fulguration and purse string sutures).

I am on hormones, received a name change and is currently living as a man
Specific_Questions: am I able to get the top and bottom surgery done at the same tome or within a week of each other if I am from out of town?

We do not recommend both procedures be done at the same time, but rather 3 months
or so apart to allow for suitable wound healing.  Just too much surgery.

Undoubtedly you may find some adventuresome surgeon who will do this, but the risks
are somewhat higher.

Specific_Questions: Hi Dr. Reed. Perhaps this will be the strangest request you get. As you can see by my information I’m a MTF who transitioned young, had SRS, etc. I have a master’s degree now and a good job – where I struggled to hold down anything as a male because I didn’t conform as one. That said, even 4 years after my SRS there is a deep and burning regret. I don’t want to live as a male, and I’m happy with hormones, implants, surgery on my face, and so on but I so badly wish I never had SRS. In short, I wish I was a woman with a penis instead of a post op. I know there probably isn’t much medical science can do (for awhile) but Dr. Reed – I feel terrible about this every single day. I’ve seen a therapist about it and while she did understand it there was nothing she could do. I have also emailed many other SRS surgeons and they either don’t respond or just say one line about how they won’t help me. Please Dr. Reed – even if you can’t help me don’t be so callous. I’m a real person who made a mistake and I admit it.  Rhonda

Simply said, you desire a phalloplasty. This is possible.
Other options including testicular implants, removal of the vagina are
all up to you.

I would certainly advise you seek clearance from 2 licensed therapists
with doctoral degrees to be sure this is not regret # 2.

Hopefully your penis is wide enough to permit 2 cylinders as we would do for genetic men. You have a choice between inflatable and semi-rigid or malleable implants.

There are pros and cons to each.

Anyway – let me know where to go from here.
