Sexual Reassignment Surgery Male To Female | FAQ re: prostate

Dr. Reed,  Do you remove the prostate during transgender vaginoplasty.  There is a history of prostate cancer in our family.  Alyssa

Dear Alyssa,

This is an often asked question about male to female transsexual surgery from both patients and referring doctors.  Surgery for prostate cancer is a called a radical prostatectomy because the entire prostate, prostatic capsule, and seminal vesicles are removed.  This may results in stress incontinence or outright urinary incontinence that is graded in the number of pads required a day.  So this is not a standard of care as far as I know, anywhere in the world.

What you should have is an annual rectal if you are under 40 and perhaps twice a year given your family history, once over 40.  Also PSA (prostate specific antigen) blood tests.  Assuming post operatively a return to a normal voiding pattern (frequency and caliber of stream), if this changes please confer with a urologist.
