Dr. Reed,  I have been a transgender woman for 6 years and have been pleasing my boyfriend with anal sex.  Is this a good idea in that I am considering transsexual male to female vaginoplasty?  Sarah

Dear Sarah,

We do not recommend anal sex for at least 8 weeks before surgery as this tends to thin out the rectum and may make it more amenable to a rectal perforation during creation of the pelvic space.  Surely after transsexual vaginoplasty we do not recommend this as i might lead to an injury called a rectal vaginal fistula.

Th symptoms of this are passing gas and stool from our vagina.  Usually this may close on its own with a high protein low residue diet for 6 weeks or so.  But why risk this?
