Many male to female transgender patients may have to lean forward to keep the urinary stream trajectory downwards.  This is correctable by re-siting the opening lower down.  Sometimes transgender voiding problems are just transient due to surgical swelling and will subside within a month or two after transgender surgery.

A inexperienced doctor may opt for antibiotics, however this is usually more anatomical than a true infection.  Surely, if you feel you are retaining urine or not emptying out properly, you need to be seen by a urologist or experienced transgender surgeon.

If  penile skin inversion vaginoplasty technique is used, penile and pubic skin has to properly be released, lest the vagina under tension strives to return to the upper area from which it came.  This drags also structures including the clitoris into high abnormal positions.
