Dear Katherine,

During male to female vaginoplasty, the prostate should not be removed.  Your question  is very timely because in the Journal Sexual Medicine Reviews appears an article entitled “Post Prostaetcomy Incontinence, How Common and Bothersome Is It Really”  When video urodynamcis are done, stress urinary incontinence was noted in 95% of patients after 4 years.

Also the potential for injury of the penile nerves which are maintained to supply the neo-clitoris is ever-present.  Please don’t mess with your potential for orgasm.  To the best of my knowledge no transgender surgeon removes the prostate during male to female vaginoplasty.
