Transgender woman wants to restore vaginal depth

Dear Dr. Reed,  Had a vaginoplasty about 3 years ago and could dilate to about 5 1/2 inches (second from the last dot).  I didn’t realize how important vaginal dilation was, became somewhat asexual, an now my depth is about 3 to 4 inches at best.  Any suggestions. Brittni

Dear Brittni,

Dilation 3 times a day after vaginoplasty or wearing a soft mold for several hours
is so important..If you re-establish proper vaginal dilating techniques, you might be able to regain most of the lost depth.  I had  a patient who was coming to me for a work list of revision items, and old me she was 4 inches and wished to go much higher.  About one week before surgery, she called an said the surgery is on, but the is no need to do a vaginal depth procedure because she is now at 6 inches. She accomplished this with aggressive dilation.  If you are not sure of what you are doing, best to see transgender surgeon who can show you, and best a friend exactly what to do.  Having a friend monitor you is very helpful initially.

Also you may consider anesthetic creams and perhaps intravaginal estrogen all prescribed by a knowledgeable transgender surgeon who will follow your progress.  There are many surgical options for vaginal depot including a skin graft, colonic vaginoplasty, the Vecchietti procedure which employs an olive boogie and an abdominal ratchet used to draw the olive inwards, and last but no least, a graft-less approach that we have employed with good results.  See:
