Can you reopen the vagina, didn’t dilate

Dear Dr. Reed,  Had a vaginoplasty done  elsewhere and did not keep up with my vaginal dilations and the vagina closed.
How would you approach this problem?  Marie

Dear Marie,

Thank you for your interest in  what we do.  Hardly a 2 week period goes by where we do not hear from someone like you with vaginal atresia.

2 years ago we wrote an article on re-opening the vaginal space, based upon a review of 87 peer reviewed article.  Our article is entitled  Non-grafted Vaginal Depth Augmentation for Transgender Atresia, Our Experience and Survey of Related Procedures and appeared in Aesthetic Plast Surg. 2015 Oct;39(5):733-44. doi: 10.1007/s00266-015-0523-7. Epub 2015 Jul 11.  Most authors said failure to regularly dilate the neo-vagina will lead to closure of the vaginal space.  We recommend 3 sessions a day lasting 15 minutes each.  Keep in mind a normal erection is about 1 1/2 inches wide.  For most patients (relating to their height) achievable depth should be between 5 1/4″ to about 6 inches.  Another alternative is wearing a soft silicone vaginal stent or dilator retained with an undergarment for several hours a day.
