Dear Janice,
Thank you for see our many sexual reassignment surgery before and after pics on  It makes no difference which surgery you have first. We do male to female sex change bottom surgery on so many patients who have already had breast implants, and then again many others with substantially large home grown breasts from hormones that have no concerns regarding their top side appearance.  I can think of no reason why one should necessarily proceed the other.  We also do bilateral orchiectomy, and some patients will opt for that first before anything else.  Sometimes decisions are made because of financial considerations, or time off from work.

If you are considering breast implants, keep in mind there are advisories which we respect to be sure the implants do not extend under the arm, that they should come reasonably close together in the midline, and generally speaking for transgender patients that the approach be submuscular.
