Had male to female vaginoplasty elsewhere, should I have a camel toe appearance

Dr. Reed,

Had male to female vaginoplasty elsewhere, should I have a camel toe appearance  Grace

Dear Grace,  The word camel toe could refer to a normal cleavage of the labia as seen in a tight inwardly cutting bikini, although most typically the bikini or panties envelop the labia as a “fused” mass.  If by “camel toe” you mean your cleavage starts too high, then this relates to how the surgery was done.  In a natal woman the cleavage under the pubis is called the anterior commissure and this normally starts below the pubic bone, certainly not well above it.

The remedy is to first undo the clitoris which also needs to be an infra-pubic structure, then undermine the skin on  either side of the camel toe and bring them together both deeply and superficially with a vertical incision.

Not uncommonly you may also have elevation of the urethral opening and this similarly needs to be brought down for a normal voiding trajectory.  The entrance way to the vagina  if too high should be closer to the anal verge so you are approachable both from the rear and on top.
