Had Sex Change Vaginoplasty, now Swelling just above my vagina

Dr. Reed,

Had a sex change (MTF) vaginoplasty abroad and when my boyfriend touches me down below, I get a bulge just above my vagina.  At times, makes penetration a little difficult. Any idea what’s going on.

Andrea

Dear Andrea,

This sounds like a classical urethral spongiosum rest to me.  The word “rest” is a medical term to describe an anatomical structure either microscopic or grossly obvious that “doesn’t belong there.”  In your case, this is left over from your penectomy.  Erectile tissue surrounding the urethra (check out your boyfriend’s bulging parts) needs to be removed.  It is fairly easy to treat via a vaginal incision.  The surgeon exposes the underside of the urethra and cauterizes the spongy tissue all around the catheter.  If there is anything else cosmetically that needs to be done as a part of your cosmetic vaginoplasty revision , there is no added charge.

A vertical incision is made very close to where the tip of the pointer is labelled “vagina” is, and the urethral spongiosum rest is ablated.
