Our transgender vaginoplasty procedure has evolved to a one stage penile skin inversion technique.  The advantage of penile skin is it is endogenously sensate and contains estrogen receptors, which could respond to oral estrogens and topical estrogen creams.  I do not wish to create a pelvic space so deep I might enter the peritoneal cavity.  As a general rule of thumb the vaginal depth at the time of surgery is your height in feet reduced to inches, and beyond that your inches converted to a fraction with 12 as the denominator.  So if your height is 5 feet 6″ your depth is 5″ (5 feet) plus 6/12 or 1/2″.  Doing the math this converts to a vaginal depth of 5 1/2 inches.

If you stand 5′ 9″ tall, your depth would be 5 and 9/12 inches or 5 3/4″ deep.  To maintain what we’ve accomplished in surgery, you do need to dilate several times a day.  This is an essential for vaginoplasty post-op care.
