Transgender woman

Dear Dr. Reed,
I am a transgender woman, now 38 years old, and have wrestled with the decision to proceed – not proceed, finally with the support of 2 therapists, realize “what am I waiting for?”  I have been living as a woman 24/7 for 2 years. Am on estradiol and spironolactone.  I may be somewhat overweight 167 for a height of 5′ 6″.  You were recommend by my therapist from Atlanta.

Sounds like you are well on your way.  As a transgender surgeon we want to be assured the likelihood of remorse is very low.  This is irreversible surgery.  Assuming you do not have any serious medical illnesses, I would recommend you lose about 10 to 15 pounds, for better height-weight conformation.  The greatest enemy to a good cosmetic result is sitting on your incision with an appreciable torso weight.  We recommend 100% use of a ring pillow for the first 5 weeks.
