Dear Susannah,

This sounds awfully familiar and was re-posed under comments.  Please provide some background.  What did the breast biopsy show?  Is there a history of breast cancer in the family?  Have any followup studies been done?   Why did you have a unilateral orchiectomy and at what age?  How is the normalcy of your remaining testicle?  Have you seen a therapist have letters of clearance.  I know many ladies like you with transgender ideation think, “why therapist?”   I am not irrational or disturbed.

This is requested because surgeons are looking for an independent professional confirmation that you will be eternally happy with your decision.  The longer you have been living as a woman (using the ladies bathroom I hope), the less likely you need “therapy”.  A letter of clearance (2) from therapists with doctoral degrees, yes (in our practice).

Then Susannah, you have a choice vaginoplasty first with unilateral orchiectomy or breast implants first.  We do not recommend both at the same time (too much surgery).  For sure you are seeking vaginoplasty depth, and a turn-on result.

In our practice patient decisions about this are 50/50.  Some patients have very good “home grown” response to estrogens.  Others are seeking that most contour enhancing curve which establishes you are a lady.
